Who we are
Agape Mobile Health (ABN 65 441 020 617) is a mobile physiotherapy practice operated by Andrew Badawy, AHPRA-registered physiotherapist (PHY0004035777), servicing Greater Sydney. This policy explains how we collect, use, store, and protect personal information — including health information — for people who visit this website, make an enquiry, or become a client.

Health service providers and this policy
Small businesses are sometimes exempt from the Privacy Act 1988 (Cth). That exemption does not apply to us — as a provider of health services that holds health information, Agape Mobile Health is bound by the Australian Privacy Principles (APPs) regardless of practice size.
What we collect
Depending on how you interact with us, we may collect:
- Enquiry details — name, phone number, email address, and the content of any message you send through the website's contact form.
- Booking and client details — name, contact details, date of birth, funding details (e.g. NDIS, DVA, Home Care Package, Medicare), and appointment history, collected when you book a visit or become a client.
- Health information — clinical notes, assessment findings, treatment history, and other information reasonably necessary for us to assess and treat you safely, collected in the course of providing physiotherapy care.
How we collect it
Website enquiries submitted through our contact form are processed by Formspree, a third-party form-handling service, and forwarded to our practice email. Bookings, client records, and clinical notes are held in Zanda Health, a cloud-based clinical practice management platform used to run the practice. Both are established third-party providers with their own security and privacy practices; we don't operate or control their systems directly, but we choose providers that take data protection seriously.
Why we collect it
We use your information to respond to enquiries, arrange and manage appointments, provide safe and appropriate physiotherapy treatment, meet our record-keeping obligations as a registered health practitioner, and — where relevant — communicate with your funding body (NDIS, DVA, Medicare, or your aged-care provider) or referring practitioners about your care.
Disclosure to third parties
We do not sell or trade your personal information. We only share it where necessary to provide your care or where required by law — for example, with your funding body or plan manager for billing purposes, with a referring GP or specialist (with your consent), or with our practice management and form-handling providers (Zanda Health and Formspree) purely as the systems we use to run the practice.
Storage and security
Your information is stored electronically through Zanda Health and, for website enquiries, Formspree — both accessed with password-protected accounts. We take reasonable steps to protect the personal information we hold from misuse, loss, and unauthorised access, though no method of electronic storage or transmission can be guaranteed 100% secure.
Access and correction
You can ask to see the personal information we hold about you, or ask us to correct it, at any time by contacting andrew@agapemobilehealth.com.au. We'll respond within a reasonable timeframe.
Complaints
If you have a concern about how we've handled your personal information, please contact us directly first so we can try to resolve it. If you're not satisfied with our response, you can lodge a complaint with the Office of the Australian Information Commissioner (OAIC).
Changes to this policy
We may update this policy from time to time as the practice grows or our systems change. The current version will always be available on this page.
